Appointment Actions

Below is a list of required documents for each appointment action requested within the JFR database.

Appointment Action Required Documents
Separation Action

 

*Note: You can request an adjunct appointment at the same times as entering a separation action.

·         Letter of resignation from faculty member (voluntary resignation)

·         Employment termination notice (involuntary resignation)

·         Patient Letter (if applicable)

·         If requesting an adjunct appointment, a new appointment action will be created in the system. Please upload the adjunct required documents to that appointment action and not the separate action. (required documents: CV, letter from dept. chair, and if applicable, letter to P&T committee for advanced rank)

Percent Time/Salary Change ·         Letter of support from the chair to the dean

·         Updated FTE allocation

Promotion (from instructor to assistant professor only) ·         Letter from faculty member requesting promotion and demonstration

·         Letter from department chair to the dean requesting promotion

·         Current CV

Secondary Appointment Instructor and Assistant Professor

·         Letter from the secondary department chair to the dean requesting appointment

·         Letter to the primary chair to the dean in support of request

·         Current CV

Associate Professor and Professor

·         Letter from the secondary department chair to the dean requesting appointment

·         Letter to the primary chair to the dean in support of request

·         Current CV

·         Letter from the primary department chair to the P&T committee

Tenure Track Change ·         Letter from the faculty member to the department chair explaining reason for request

·         Letter from the department chair to the dean requesting pathway change

·         Current CV

·         Pathway change request change (template found within JFR system)

·         Update FTE allocation

Departmental Transfer ·         Letter from the requesting department chair to the dean is support of the request

·         Letter from the current department chair to the dean supporting the transfer

·         Current CV
